SFESurvey v1.0 will allow you to create multiple-choice quizzes or surveys online with a twist. Rather than having one correct answer, and multiple incorrect distractors, each possible answer is weighted or assigned a certain number of points. The instructor/test creator can then assign various comments or scores to different ranges of raw scores.
There is a working model of the program here and a demo of the admin
section here (Use the username "admin" and the password "test" to log in
and create a survey or test.)
Tested on Windows 98 (with Tinyweb web server) and Unix/Linux (Apache
web server). If you get it to run on another
platform, please let me know.
The quiz taker's score and any additional fields you choose (Name, Age,
Gender, Occupation, Favorite Color -- whatever you like) are saved to a
score database, but there is currently no option to view scores or score
statistics.
You must be able to execute cgi programs on your web server. If you're not sure whether you have permission to do this, or are not sure how to configure the files, please ask your system administrator, or check the web pages of your web hosting provider for Help or Support information. For a free web hosting provider who supports CGI scripts, I recommend HyperMart or Virtual Nine Network.
You can view the individual files below, or download the package in a gzip-compressed tar file. On a unix system, typing 'gunzip sfesurvey.tar.gz' then typing 'tar -xvf sfesurvey.tar' will usually work. If you're not very familiar with Unix, you may want to ask someone who is to help you.
If you have MS Windows, there is a program called WinZip (available at TuCows which will uncompress and de-archive tar/gz files. I understand that the latest version of Stuffit for the Macintosh will also handle tar.gz files.
